For clinics and community teams
Patient information and signposting
Add FND Connect resources to existing patient information and discharge processes, without transferring identifiable patient data.
Purpose
Enable healthcare services to incorporate appropriate FND Connect resources into their existing patient information and discharge processes.
Who this is for
GP practices, neurology departments, hospitals, rehabilitation services and community healthcare teams.
What FND Connect can provide
- Patient information leaflets
- QR-linked educational resources
- Links to the Start Here pathway
- FND Passport information
- SeizeControl introduction materials
- Details of community and virtual support groups
- Accessible information for families and carers
How it works
A healthcare professional identifies a patient who may benefit from additional non-clinical information. They provide an FND Connect resource or a page link. The patient independently decides whether to use FND Connect. No identifiable patient information needs to pass from the healthcare organisation to FND Connect.
Requirements and boundaries
A downloadable Professional Resource Pack is available immediately. Organisations can register interest. Ordinary public information does not need a complex agreement. A formal agreement is only required where the relationship warrants one.
