North Bristol NHS Trust’s neuropsychiatry service is based at The Rosa Burden Centre, Southmead Hospital. We have an outpatient clinic in which we carry out assessments and provide guidance. We provide outpatient psychological treatment (CBT) for suitable patients. The neuropsychiatry team is a mix of professionals from different disciplines including: Doctors in psychiatry and neurology. Nurse therapists trained in physical and mental health. Psychological therapy. Physiotherapy. Occupational therapy. We work with patients who have a mixture of different problems including: Mental illness associated with neurological diseases. Epilepsy and non-epileptic attacks. Functional neurological symptoms. Neurological sleep disorders. We aim to provide high quality specialist assessment and treatment plans that incorporate expertise in both physical and mental health. Being a part of the neuroscience department, we work closely with our colleagues in neurology and neurosurgery. We also provide neuropsychiatry support for the mental health liaison team for inpatients in other wards at Southmead Hospital. Our outpatient clinics take referrals from across the south west from general practitioners, neurologists, neurosurgeons and psychiatrists. Our inpatient service is situated within the Rosa Burden Centre. We have two inpatient beds for patients with neuropsychiatric conditions, and two inpatient beds for assessment of sleep disorders. For patients who have tried all the other available treatment and are still very affected by their symptoms, we have a three week inpatient rehabilitation program. Our inpatient program is “multidisciplinary” meaning it involves assessment and treatment from professionals with different expertise: • Nursing staff trained in physical and mental health • Psychological therapy • Physiotherapy • Occupational therapy • Psychiatry Through 1:1 assessments with different professionals, and weekly team meetings involving the patients, we aim to get a good understanding of your symptoms, how they affect you and how you understand them. Part of the assessment involves us getting to understand you as an individual, because these symptoms are complex and can be affected by lots of different aspects of a person’s history. We will challenge you to try different techniques and learn new skills, to enable you to manage your symptoms more effectively. By the time the three weeks are over we hope that your symptoms will have improved, your ability to manage the activities of daily life will have improved, and you will have confidence that you can continue to recover at home. Functional Neurological Disorder (FND) clinic is an outpatient clinic run by a small multidisciplinary team (MDT) of a Psychologist, Occupational Therapist and Physiotherapist. We aim at providing education around understanding the functional diagnosis and symptom management. The treatment is based on a self-management approach to alleviate the distress from the symptoms experienced and practicing strategies/ exercises to retrain brain allowing restoring the disrupted function of the body. Once a patient is accepted within the service, they are seen in the clinic by the team for detailed MDT assessment. Patients often receive individual and joint sessions from each discipline based on their needs and these can be physical appointments in the clinic. The clinic is situated at Danesbury Neurological Centre, Welwyn. The treatment package is of 5-6 sessions in total. However, patients may receive more sessions if needed to complete their treatment.

The aim of the programme is to support patients to understand their condition and learn to independently self-manage their symptoms using taught strategies. Treatment for FND Patients The NHNN has a long-established treatment programme for patients with FND within the Neuropsychiatry Department. Patients may be referred to our MDT Outpatient Clinic for assessment of suitability for treatment. The referring doctor needs to have made a positive diagnosis of FND and to have explained this to the patient. We do not provide a diagnostic service for this condition. Patients will be seen in-person or assessed via telephone by a Neuropsychiatrist or CNS-Neuropsychiatry and one or more therapists (Cognitive Behavioural Therapy, Occupational Therapy or Physiotherapy). FND Services Provided We currently provide different pathways for treatment, dependant on the outcome of the assessment: • Inpatient Treatment: 4 week programme with treatment sessions taking place Monday to Friday, with weekend home-leave. • Outpatient Treatment: 5 week programme, attending two consecutive days per week for the period (Monday/Tuesday or Thursday/Friday for the duration of the programme). • Outpatient CBT Patients referred to our service have complex-needs and may have a combination of symptoms including but not exclusively motor-disorder. We are not a Pain or Fatigue Management Service. Our inpatient and outpatient programmes offer similar input of Cognitive Behavioural Psychotherapy (CBT), Specialist nursing (Mental Health and General), Physiotherapy and Occupational Therapy. Patients not willing to consider psychological therapies as a part of their treatment will not be accepted for our programmes and are advised to seek community therapy services referrals via their GP. Guided Self-Help (GSH) Course accessed on-line with telephone support, specifically for patients accepted onto the inpatient programme. St George’s provides assessment and treatment for FND across a variety of inpatient and outpatient settings. We routinely diagnose patients with FND in outpatient clinics, as well as on the acute medical/neurology wards at St George’s. Patients move through our pathway receiving rehabilitation for FND either as an outpatient or after being admitted for inpatient neurorehabilitation. • We run weekly neurology-led outpatient clinics for people with suspected FND or those who have an established diagnosis of FND who are seeking treatment. The neurology team who run these clinics have a specialist interest in FND as well as in neurological disorders in general. Our clinic predominantly sees people with functional movement and sensory symptoms. People with possible or confirmed dissociative seizures/non-epileptic attacks are usually seen with our specialist epilepsy service by consultant colleagues who link through to our treatment services and who can conduct appropriate specialist diagnostic assessments when needed. Our aim is to provide people with a diagnosis which is explained clearly, and then with advice regarding treatment. We then try to either provide this treatment within our own team or to refer on to other services to provide treatment. • We run a monthly workshop for people newly-diagnosed with FND. Neurology consultants and others who have made the diagnosis of FND can refer people who are interested to this workshop. The workshop consists of educational talks about FND from Professor Edwards, Dr Niruj Agrawal (consultant neuropsychiatrist), Ms Kate Holt (specialist neurophysiotherapist) and Dr Sarah Cope (neuropsychologist) and a person with FND who has been through treatment in our service. There is then an opportunity to ask questions and share information. • A proportion of people with functional movement problems are suitable for intensive brief physiotherapy treatment - Dr Glenn Nielsen (specialist neurophysiotherapist) and Ms Kate Holt deliver this treatment. We have developed this treatment over the past 5 years, publishing outcome studies and have recently been granted funds from the National Institute for Health Research to lead a multicentre UK clinical trial of this intervention. • Specialist neuropsychologists with specific expertise and interest in FND offer 1:1 and group psychological therapy. Much of this work relates to the treatment of dissociative seizures/non-epileptic attacks but some people with other FND symptoms can also benefit from such intervention. Referrals by GP.

The inpatient neurorehabilitation programme is based at Queen Mary’s Hospital, Roehampton as part of St George’s Hospital NHS Trust. It provides assessment and treatment for FND across a variety of inpatient and outpatient settings. We routinely diagnose patients with FND in outpatient clinics, as well as on the acute medical/neurology wards at St George’s. Patients move through our pathway receiving rehabilitation for FND either as an outpatient or after being admitted for inpatient neurorehabilitation. The inpatient neurorehabilitation programme is based at Queen Mary’s Hospital, Roehampton. Patients are initially assessed at our multidisciplinary team assessment clinic at which point we identify their suitability for inpatient rehabilitation, review their history and diagnosis, and identify clear therapy goals. We have 8 beds in our neuropsychiatry inpatient programme at Queen Mary’s. 4 Beds are for patients with acquired brain injury / degenerative conditions. 4 Beds are for patients with FND. The treatment approach is multidisciplinary and professionals include: Physiotherapist, Occupational therapists, Neuropsychologists, Speech and Language therapists, Specialist Nursing, Neuropsychiatry In addition to these disciplines, we have regular input from rehabilitation, urology and neurology consultants. A combination of the above therapies is tailored to each patient’s needs. This holistic approach seeks to treat every patient using the bio-psycho-social model. Therapy goals are different for each person and are identified by patients. Often these include improving mobility, increasing independence with specific tasks, managing pain, as well improving fatigue and mood management. The average length of stay on the ward is between 6-12 weeks. Prior to discharge an on-going management plan is formulated to support the transition back into the community. This may involve referral to additional services such as community rehabilitation and outpatient follow up. Referral by Neurologist.
